A day in the role of a Clinical Director of Care (CDOC) at Cooksville Care Centre.

The Clinical Director of Care is responsible for the overall provision of care and services to residents, coordination of the quality management program and acts as a resource and coach to staff in various clinical areas.

This role specifically will serve as an inhouse expert on identification and management of responsive behaviours. This dedicated lead will function in a specialty role providing behavioural support expertise to older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions. The CDOC will work as a member of the leadership team within the long-term care home, and in partnership with the Behavioural Support Long Term Care Team Lead and Assistant provide build the capacity of all staff within the home

In conjunction with the Nursing Supervisor, Director of Care and Staff Development Coordinator ensures compliance with legislative requirements (Ministry of Health and College of Nurses of Ontario) and acts as a coach to the nursing staff to ensure best practices are followed.

As a Clinical Director of Nursing, you will be part of our Nursing team, and you will report directly to the Director of Care. This position will be a 12-month temporary contract with possibility of extension. Candidates must be available to work 8:30 to 4:30 p.m., but flexibility will be required (including some evenings and weekends).

This position is responsible for but not limited to:

  • Maintain a resident-centered approach in assessment, planning, provision and evaluation of resident care
  • Liaise with Program senior leadership
  • Liaise between the family and staff
  • Coaches other staff on behavioural care plan for residents
  • Site visits to community partners and hospitals to assess for new admissions
  • Reviewing and approving applications from the LHIN for new admissions
  • Liaise with outside behavioural care providers
  • Liaise with long term care home physician(s)
  • Assists in leading/participates in development of behavioural care plan
  • Conducts behavioural assessment for residents upon admission to long term care home
  • Provides case management for behavioural residents
  • Updates behavioural sections of Resident Assessment Instrument (RAI) and Mini Data Set (MDS) care plan
  • Leads implementation of care plan recommendations
  • Facilitate care conference for all resident’s preadmission, 2-week post admission, 6 week post admission
  • Monitors and evaluates care provided
  • Assists in the leading education and knowledge transfer to all staff on preventative behaviour management
  • Participates in nursing research, resident satisfaction and continuous quality improvement initiatives (e.g. Residents’ First)
  • Maintains active membership in appropriate professional associations.
  • Perform all other duties as assigned
  • Applies current knowledge regarding resident needs, gerontological principles, nursing care, general health maintenance and promotion
  • Provides ongoing assessment of resident health status in accordance with defined policy and procedures.
  • Contributes to the evaluation of nursing care by comparing resident status to expected outcomes and goals so the RN can modify the care plan as appropriate.
  • Demonstrates self-awareness in realistic appraisal of personal practice, identifying strengths, weaknesses and plan for professional development.

Qualifications

  • Registered Nurse (BScn or Diploma)
  • Registered as an RN in good standing with the College of Nurses of Ontario
  • Prior experience dealing with clients with responsive behaviours in long term care
  • Current Standard First Aid and CPR certification
  • Training, or the ability to obtain training in, P.I.E.C.E.S., Gentle Persuasive Approach, U-First!, Montessori, etc.
  • Three (3) years recent experience as an RN in long term care Supervisory role considered an asset
  • Experience with quality improvement processes and policy and program development considered an asset
  • Must not be working at another facility due to the pandemic
  • Must be comfortable working with COVID-19 positive residents
  • Must be willing to get swabbed for COVID-19
  • Must have a clear vulnerable sector screen police record, negative TB test results, evidence of flu shot, must be able to provide three work/professional references (phone numbers and emails)
