Under the direction of the Registered Practical Nurse/ Health and Wellness Manager, delivers medication and treatment as per policy and procedure. Delivers care to residents based on individualized care plan, policies and procedures, and established standards of care.
Has obtained a pharmacy UCP course certificate. Written and verbal fluency in English is required. Has proficient computer skills (i.e. Word and Excel). Preference is given to individuals with 2 or more years of experience working with seniors. A resident-oriented individual, whose visions and values align with those of Diversicare.
This position requires you to work evenings and weekends when needed.
Participates in the Verve Quality Improvement (VQI) program.
Administers medication and treatments in accordance with current approved standards of care, and UCP policies and procedures.
Provides care to residents (i.e. cognitive, social, and emotional support), according to their plan of care and in a manner that promotes comfort and safety (i.e. in accordance with the standards of resident care in nursing manuals).
Supports residents’ optimal level of functioning in their activities of daily living, while respecting their rights and choices.
Assists in monitoring and evaluating the outcome of specific interventions regularly and offers suggestions for improvement.
Reports any deviations in the plan of care and reason for it to the immediate supervisor.
Completes nursing documentation accurately and legibly according to policies and procedures. Maintains health records in compliance with legislations.
Complies with the Occupational Health and Safety Act, Workplace Safety and Insurance Act, and Workplace Hazardous Material Information Systems. This includes recognizing health and safety hazards, reporting incidents and/or any changes to resident’s daily life, fulfilling responsibilities under the applicable legislation, as well as participating in in-services and fire drills. Complies with other infection control protocols as directed (i.e. outbreak management).
Complies with all relevant corporate policies and procedures.
Completes all required mandatory training/education.
Performs other related tasks as delegated by the Registered Practical Nurse or Health and Wellness Manager.
