Position Title: Team Leader
Responsible To : Coordinator and/or Designate
Team Leader may be required to undertake all the duties as per the Community Support Worker level 2 position description in addition to the duties as described below.

Summary of the broad purpose of the position
Responsible to the Program Designate for the leadership of a team of staff who together ensure the provision of direct care services to individuals with a disability, by providing a range of quality community/residential based services that meet individual needs.
Act as a role model for other team members which reflects the principles, policies, and procedures of CommunityAim
Accountability and reporting/working relationships
The Team Leader is responsible to the Program Coordinator and/or Designate. If there is no designated Program Coordinator and/or Program Manager then the Team Leader is responsible to Designated Program Manager.
Special Conditions
- Must be willing to work over a 7 day schedule, including weekends, evenings and may be required to undertake overnight shifts as needed.
- At times work outside of normal scheduled hours will be required.
- Must be willing to carry on-call cell phone as part of on-call schedule if requested to do so.
- Hiring to the position is subject to satisfactory criminal record check clearance and pre-employment screening.
- Must hold a current First Aid/CPR certificate at a satisfactory level that meets CommunityAim policy.
- Must hold a current Alberta class 5 license.
- Access to own roadworthy vehicle with appropriate insurance that meets CommunityAim policy.
- Must be physically, emotionally and medically able to lift, transfer, undertake manual handling and be able to perform any aspect of care to the individual(s) supported as required.
Activities/daily responsibilities
- Assisting in assessing the needs of individuals across all life domains.*
- Assisting in the design, delivery, review and evaluation of service delivery programs and activities offered to clients*
- Providing support in a way that is sensitive to spiritual, cultural and language issues*
- Promoting the dignity and positive image of individuals within the community.*
- Directing and providing direct care in a supportive and safe environment.*
- Directing and providing household management schedule to ensure daily weekly tasks are completed ie yard care, household cleaning based on daily, weekly yearly obligations that are required in any home.*
- Directing, providing and ensuring that a monthly menu plan is developed and is in accordance with the Canada Food Guide. *
- Assisting individuals with the management of their finances (ie, household budgets, and individual budgets) including negotiations with Guardian Trustee and the Public Trustee.*
- Assisting in the developing, coordination, implementation and ongoing monitoring of annual service plans.*
- Maximizing opportunities for individuals to participate in and contribute to their community.*
- Supporting and encouraging family relationships and friendships.*
- Maintaining cooperative relationships and liaising with other agencies as appropriate.*
- Organizing and participating in individual-based meetings, staff meetings, and Guardian meetings.*
- Developing and implementing positive behaviour plans in conjunction with the Program Coordinator/Manager.*
- Ensuring that confidentiality is maintained at all levels.*
- Identifying risks and participating in risk management.*
- Providing opportunities for individuals to make informed choices.*
Ensure client health needs are met by:
- Liaising with health care staff e.g. GP, specialists, as appropriate. *
- Maintaining contacts with and utilizing community based health care facilities.*
- Initiating, monitoring and implementing any Health Care plans that may be in effect/appropriate*
- Ensuring accurate recording of the administration of prescribed medications and other health related documentation.*
- Administering prescribed medications in accordance with CommunityAim policies regarding handling and administration of medication.*
- Ensuring the implementation of CommunityAim Health care policies.*
- Maintaining the domestic environment in a manner that promoted optimum health conditions.*
Ensuring staff are supported in their roles and are able to effectively carry out their duties by:
- Supervising and supporting individual staff members.*
- Providing verbal and written feedback to both staff, coordinator, manager and Senior Program Manager in a manner that promoted ongoing staff development.*
- Ensuring that staff is aware of, and adhere to, relevant organization policies, procedures, and work practices.*
- Providing a positive work environment and a team approach.*
- Provide support to ensure that appropriate documentation and administration are accurately maintained within the individuals household.*
- Identifying opportunities for training and development*
- Acting as positive role model *
- Providing initial point of contact for staff.*
- Assist in the orientation of new staff within the program.*
Ensuring that the service operates in a systematic manner and within the budget by:
- Ensuring that attention is given to accurate recording and reconciliation of household and individual budgets and maintaining the budgets within the allotted amounts set out.*
- Ensuring that the required administrative records are accurately maintained and reported on.*
Contribute to the organizational development of CommunityAim by:
- Providing services that are consistent with the principles, standards, mission and vision of CommunityAim.*
- Providing qualitative and quantitative data to Coordinator and/or Program Manager about the programs offered through CommunityAim. *
- Contributing to the enhancement of a positive and progressive organizational culture that is supportive of change to better meet the goals of clients and the organization.*
- Operating ethically, fairly, and within statutory, legal and contractual requirements.*
Occupational Health and Safety
- Follow the procedures manual and policies pertaining to safety and report all health and safety matters to the Coordinator/ Program Manager immediately.*

- Take necessary precautions to avoid compromising the health and safety of others.*
- Use any equipment provided for health and safety purposes and follow the E-T/AE policy.*
- Obey any reasonable instruction that has been given in relation to health and safety at work.*

- Report any unresolved concerns or problems, including inter-staff conflicts to the Senior Program Manager.*
Educational / Vocational Qualifications
- Certificate, Diploma and/or Degree in a field related to disabilities and a minimum of 3 years experience in direct service provision.*

- Minimum 5 years experience in a front-line type role in direct service provision.*

- Proven ability to lead a team including facilitating team processes, management of issues, provision of support and role modeling.*
- Proven ability and experience in developing positive behaviour support plans if the program requires such plans.*
