HOURS: Days, 8 hours, Monday-Friday (four days per week) (Subject to change in accordance with operational requirements)

JOB SUMMARY:
With guidance, support the implementation of Health Equity portfolio’s goals, objectives and deliverables. The focus will be on projects and initiatives that meet the diverse needs of, and equitable access to quality health care for, patients/families.

KEY RESPONSIBILITES:
Reporting to the Team Lead, Health Equity, the candidate will:

Support the delivery of the Health Equity portfolio’s goals and objectives
Support the implementation of Health Equity’s initiatives and monitor impacts, e.g.
Work towards equitable access to quality and risk-free care for marginalized patient populations including:
Strengthen access to accurate communication for patients who are limited English proficient and/or require Sign Language
Tracking and analyzing interpretation usage and costs and measuring return on investment
Organizing learning opportunities/developing tools & resources to build staff capacity related to culturally competent care
Strengthen and monitor partnerships with community organizations/networks that can help meet the diverse needs of Osler’s high need/high risk patient populations for transition into the community
Improving data collection/analysis for evidence-based decision-making
Provide diversity/health equity consultation/subject matter expertise to front-line staff and committees/employee resource groups
Support health literacy/patient education/self-management initiatives
Support the role of Osler within the Ontario Health Team
Support the Team Lead and Director, Health Equity, in all projects/initiatives

QUALIFICATIONS:
College Diploma or University Degree in Health Management or related field
Minimum three years of experience in health equity and working with high needs/at-risk communities
Experience in the health sector, particularly inequities, disparities and barriers to health care
Formal training and experience in diversity and health equity
Formal training and experience in project management
Formal training and experience in data collection/analysis
Must be able to meet deadlines with minimal supervision

Excellent knowledge of:
Diversity/health equity/cultural competency principles, concepts and sensitivities
Analysis and interpretation of data related to health inequities and disparities
Relationship management
Communication - written and verbal
Preparing and delivering presentations
Computer skills including PowerPoint and Excel
