Reporting to the Director, Digital Innovation and Transformation, Central Region; the Specialist, Digital Health (Change Management) will play a key role in the successful implementation and adoption of technology in the long-term care homes and primary care provider offices within the Central Region. The Specialist will apply a structured project and change management approach and methodology to support providers in the adoption of digital technology to improve the health of residents and optimize healthcare resource utilization. The Specialist will need to build and support a variety of stakeholder relationships work closely with long-term care homes and primary care providers to advance the implementation of digital solutions. It will also be important to ensure alignment of work with local digital health priorities so as to not introduce competing models of change.
Here is what you will be doing:
Primary point of contact for assigned project(s)
Work closely with key members of each deployment site (e.g. long-term care home staff or primary care providers) to apply a structured change management approach and methodology to advance the implementation plan, ensure reporting of key metrics for evaluation
Work closely with Ontario Health Central Region program leads and other implementation team members to ensure alignment of activities to broader program vision, objectives, and benefits realization
Work closely with third party vendors and stakeholders to ensure education delivered to care providers is effective and sustainable
Be an active and visible coach to clinicians and stakeholders
Work closely with long-term care home staff and primary care providers to build capacity in adopting new technologies and clinical workflows/pathways
Work closely with long-term care homes and primary care providers to build capacity in managing measurement systems to track adoption, utilization and proficiency of users
Identify potential risks, points of resistance, performance gaps and develop appropriate mitigation activities. Work with partners to remove barriers to change
Work with Ontario Health Central Region team to support evaluation to capture the organizational and clinical value of digital projects
Document lessons learned and present findings in a logical and easy-to-understand manner
Create and enable reinforcement mechanisms and celebrations of successes

Here is what you will need to be successful:
Education and Experience
University degree in a Clinical Discipline, Healthcare Administration, Business Administration or a related discipline
Minimum of 2 years of experience in clinical healthcare delivery and/or supporting digital health initiatives or within a health care capacity (long-term care is preferred)
Registration with a professional regulatory college (e.g. nursing) is preferred
Solid understanding of clinical processes (long-term care preferred)
Keen awareness of integrated care and the evolving Ontario Health Team model
Well-developed understanding of the impact of technology on patient service delivery
Previous experience with enabling technologies and digital health within Ontario is preferred
Knowledge of provincial and local health-care priorities, initiative, and issues
Knowledge of Ontario Health region business strategies, objectives, priorities and programs, and related communications priorities and plans
Knowledge and understanding of the Ontario Health region stakeholders as well as overall issues and priorities within the healthcare sector
Knowledge of Ontario Health region administrative practices, policies and procedures, especially around confidentiality and release of information
Knowledge of technologies employed in long-term care an asset
Experience in an Ontario Health region or a legacy LHIN is an asset
Experience with staff education (e.g. clinical educator) an asset
Experience in data reporting and management an asset
Prior experience in evaluation or research an asset
Change Management Professional certification an asset
Previous experience with enabling technologies and digital health within Ontario is preferred
Proficient in SharePoint, Microsoft Office, Microsoft Project

Employment Type: Temporary Full-Time (12 months with benefits)
Employment Location: Central Region, Ontario (currently virtual; subject to change). All applicants must be a resident of Ontario to be considered for roles at Ontario Health.
