Humber River Hospital, located in the new Wilson site, a state-of-the-art, fully digital facility, is seeking a dynamic, evidenced-based Patient Flow Manager. This experienced individual will coordinate and facilitate the seamless flow of internal and external patient admissions, transfers, and discharges that ensure “the right patient is placed in the right bed at the right time”, and maximize the utilization and allocation of patient beds and clinical resources within our 24/7/365 clinical operation.

Reporting Relationship: Program Director, Emergency Services
Hiring Salary Range: $48.419-51.44
Coordinate patient flow from internal and external patient admissions, transfers, and discharges (e.g. coordinate areas of care in Emergency Department (ED), bed allocation team, portering, housekeeping) to ensure all patients are admitted and referred to the most appropriate services for their clinical needs, and resolve patient flow issues as they arise.
Coordinate and redeploy staff to achieve appropriate skill mix based on patient safety, departmental coverage needs, and associated risks.
Collaborate with inpatient managers to identify process improvement strategies to improve the current inpatient system and reduce patient length of stay (LOS).
Define and prepare reports to inform management, physician chiefs, and administrative team of patient activity and bed utilization to manage surges in patient volumes.
Ensure staff completes Quality/Risk Management (QRM) of Automated Endoscope Reprocessors (AER) to develop quality improvement initiatives and risk mitigation strategies.
Supervise after-hours clinical services to act as night supervisor and primary crisis contact for all clinical activity areas in hospital (e.g. developing discharge plans, resolving family conflicts, emergency services).
Identify and communicate clinical concerns or systems issues that occurred during evening or weekends to manager for action.
Facilitate and escalate in order to complete patient flow with a focus on clinical activities.
Expedite, prioritize, synchronize and mediate clinical issues, barriers.
Back up PSC as needed.
In collaboration with ALC/Complex Care Coordinator trend delays and support quality improvements with leadership from clinical, professional practice, transformation, and utilization committee.
Review and respond, in real time, to clinical indicators for patient care delays.
Track, prioritize and facilitate key discharge related workflow activities.
Address conditional discharges (MI, Lab, etc.) orders.
Track and prioritize ancillary (PT/OT) and physician consult.
Review and respond to clinical indicators for patients falling off clinical pathways.
Liaise with MI Coordinator regarding clinical priorities.

Review/Approve Observer requests requiring Patient Flow Manager’s approval
Approve overtime, agency use after hours
A Baccalaureate Degree in Nursing
Current registration and membership in good standing with the College of Nurses of Ontario (CNO)
Membership with the Registered Nurses Association of Ontario (RNAO)
A minimum of 7 years of clinical experience in acute care with strong experience a community based hospital.
Current/previous Emergency experience
Exceptional analytical and information-seeking skills that contribute to effective decision-making and strategic planning and execution of action plans that deliver results and motivate individuals for greater performance excellence
Ability to work with a diverse stakeholder group with demonstrated facilitating and coaching clinical areas on change initiatives
Self-directed, courageous, and highly motivated with excellent interpersonal and communication skills
Experience managing competing demands in a fast-paced, multi-site, multi-disciplinary health care environment
Proven risk management strategies
Excellent attendance and discipline free record required.
Location: Humber River Hospital – Wilson Site
Hours of work: Rotating shifts, 8 or 12 hours, days/evenings/nights/weekends/stats
