We are looking for a Registered Nurse to join our Multidisciplinary Clinical Supports Program (MCSP) at London Family Court Clinic (LFCC). As a Registered Nurse, you will use your specialized knowledge and skills to provide health care and education to young people living in group home settings. As part of the MCSP team you will also provide consultation and support to the team’s mental health clinicians and the stakeholder group home staff.

In this role, you will:
Provide comprehensive health care and education for adolescent group home residents, for the purpose of supporting their overall well-being and mental health treatment needs

  • Coordinate young people’s health care with other members of the MCSP team, such as the psychiatrist, social workers, psychotherapists and psychologist
  • Monitor adherence and response to health care interventions
  • Liaise with community services to obtain health histories/records and facilitate immunization and other health care needs
  • Contribute to health care programing within the group home setting, including medication training for group home staff
  • Develop and provide educational groups for the purpose of promoting health and preventing future health issues for youth
  • Establish and maintain positive working relationships with other staff members and external stakeholders
  • Share your knowledge and expertise with staff members
  • Practice in accordance with the College of Nurses of Ontario Practice Standards and Ministry expectations as well as the policies and procedures of LFCC and the stakeholder sites

Qualifications:

  • You must be a Registered Nurse with the College of Nurses of Ontario and be in good standing
  • Mental Health Certification is an asset
  • Driver’s license and a reliable car are required
  • Knowledge of OSHA, FDA, and HIPAA compliance

Nursing and Mental Health Knowledge and Experience:

· You have in-depth knowledge of mental health nursing theory, community resources, evidence-based practices and adolescent treatment needs

  • You can interpret and apply relevant legislation (such as the Child, Youth and Family Services Act, Mental Health Act, Regulated Health Professions Act, Health Care Consent Act, Personal Health Information Protection Act)
  • You have knowledge of the interplay between mental and physical health and experience in working with young people
  • You can elicit, interpret and evaluate information from patients and other sources and draw conclusions to contribute toward interdisciplinary care plans
  • You can consult on emergency situations and implement appropriate medical and therapeutic interventions
  • You have experience and enthusiasm for prevention work that includes the development and provision of psycho-educational group programs

Effective Communication Skills:

· You can present information in a clear and concise manner as well as adapt your communication skills to provide information and health teaching to a diverse population of youth and professionals

· You are a skilled group facilitator and enjoy educational approaches to prevention

  • You can work collaboratively with others, such as the inter-disciplinary MCSP team, group home staff and community agency professionals
  • You can effectively advocate for and encourage youth to participate in their health care and to work toward their goals
  • You have oral and written communication skills to clearly communicate care and progress through documentation, including progress notes, written care plans, site documentation.

Hours of Work:

This is a permanent .7 FTE position that has flexible weekday hours. (26 hours/wk)
