The social worker is part of the Clinical and Therapeutic team. He works in collaboration with the multidisciplinary teams and is part of the discharge team. Acting as a discharge planner the social work will work with the patient, his family and with the multidisciplinary team in planer. Provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. The social worker assists with the planning for those patients and their families who pose a challenge in their return home or require admission to a LTCF.
Certificate of registration from the Ontario College of Social Workers and Social Service
Bachelor of Social Work (BSW)
Experience in a hospital setting
Education and experience in conflict management
Possess a working knowledge of both official languages.
Knowledge of community partners
Previous experience in placement to alternative institutions: LTCF, residences, supportive housing.
Experience with older people
Experience and ability to work as a multidisciplinary team.
Leadership, abilities for problem resolutions
Demonstrated interpersonal and communication skills.
As a pre-condition of employment, all new HGH employees will be required to obtain and provide a satisfactory Police Records Check dated within six (6) months from the date of issue.
