Position Brief

The PMH Program Coordinator role will manage resources, coordinate and support activities related to family physician practice transition towards Patient Medical Homes. This position is a full-time employment position with a schedule that involves work in the Division’s office, throughout Richmond and at times that can include early morning and evenings. Our office is located in central Richmond close to a Skytrain station. The role of Program Coordinator reports to the Executive Director of the Richmond Division of Family Practice.

Job Tasks:

  • Support the sustainment and spread of our Neighbourhood Networks initiative though coordination of clinical and practice resources, panel management, member engagement and participation in planning efforts to expand and enhance the networks;
  • Support the MOA Network through planning and delivery of services;
  • Coordinate our attachment program to match patients seeking a family physician;
  • Maintain active and positive relationships with relationships with family physicians, MOAs, specialists, clinics, and community services within Richmond;
  • Review and update physician information on a regular basis to ensure that the appropriate contact and referral details are on Pathways;
  • Support registration and training on Pathways for physicians and MOAs;
  • Create our bi-monthly member newsletter with content generated by our team, ensuring accuracy and an eye for detail
  • Prepare reports, presentations and materials for various audiences including physicians, MOAs and community partners;
  • Support member events and meetings through participation in planning, execution and evaluation;
  • Work closely with other members of our team including Physician Leads in the support of our members and to achieve the strategic and programmatic goals of our organization
  • Create project charters and plans, monitor timelines and deliverables

Job Requirements/Qualifications:

  • Bachelor’s Degree in Health Administration, Business Administration, or a Health Science
  • Minimum of 2 years experience working in a healthcare setting, preferably primary care
  • Project management skills
  • Effective and clear communication skills (written and oral);
  • Ability to multitask, take initiative, and prioritize;
  • Ability to maintain a high level of professionalism and use tact/discretion in all interactions;
  • Works well as part of a team but maintains focus and initiative to work independently
  • Available to attend and participate in division events or meetings at times than can include early morning, 0730 start, or that end at 21:00

Job Type: Full-time

Salary: $56,000.00 to $58,000.00 /year
